Because free version of MailChimp allows to create only 1 form. Do you have premium version to create multple forms and want to use different forms for the different areas or it will be the same form but in different places?

Problem caused by your cache, once I enable Dev Mode and disabled cache http://prntscr.com/fdeu0d 500 error disappears. And any error on frontend. Do you have any errors in server logs? Do you have any advanced settings for your cache?

1) I have added the following code in Custom CSS to change the colors. You can change it according to your needs

2) If you want to change the color of inputs go to Theme Options > Styling > Content > Form inputs color

That demo uses old version of WC plugin that’s why the order of the checkout fields is different.
WC plugin changed the order of fields after 3.0 update. To change the styles of the State and country fields add the following custom in Theme Options > Styling > Custom CSS
