Hello,
WooCommerce does not have options to add any custom fields to checkout page by default. You can add only create account, ship to billing address and terms & conditions checkboxes using WooCommerce settings. If you need any other custom checkbox then try WooCommerce Checkout Manager plugin.

We have checked your site and compared shop page speed with the default WP theme (Storefront) and results are bad https://tools.pingdom.com/#5c0cd4a47b400000
We installed the cache plugin and it improved speed a lot https://tools.pingdom.com/#5c0cd35691400000 but we would recommend you to contact the hosting provider and ask to fix the site speed from their side (server response time etc) when the cache is disabled because even the backend of your site is slow.
Is everything ok once you disable the cache plugin?

Checkout page is the default WooCommerce page and WooCommerce plugin does not display product thumbnails on the checkout page, only on the cart page. If you want to change this you can do this only by additional customization, there is no such option in our theme. Additional customization request you may submit here.
