Product categories filter in sidebar has no heirarchy despite being enabled

The menu widget in the sidebar simply shows a majority of categories and not in their parent/child status (although it is in the correct order as if you disable show hierarchy it reverts to sorting alphabetically).

This is really messy and confusing – there doesn’t seem to be anything other than the de facto widget? How did I break this!

Should be

Essentials
– Condoms & Protection
– – Coloured & Flavoured
– – Extra Large
……..
