Replicate the contact page from Electronic Xstore

Hi,
how can I make the exact contact page from the Electronic store?
1. I can’t put that contact info over the map
2. I don’t know how to set the contact form to be wider, to occupy 1 whole column.

Or edit the page example that we have created for you – Contact Us page at the pages list.
