And I don’t have a sign-in page, only a login. Where is the sign-in page?

Hello @Pedro Monteiro,
1. You can use the shortcodes of the fields you used in your form (http://prntscr.com/vk054l) and place them in the areas you want to show (http://prntscr.com/vk0401). Since there is no element in your form with the shortcodes [your-name] or [your-subject], you will only get the shortcodes.
2. If you would like the registration form to appear in the Login form, please navigate to the Dashboard >> WooCommerce >> Settings >> Accounts & Privacy and ensure that the option to create an account on the “My Account” page is enabled.
