Form is not Displaying on Frontend whereas backend is ok on product page form popup

greetings!

we hope you are in good health.

The form on the single product page is not displaying properly on the frontend, even though it appears fine in the backend and Elementor editor.

Hello @khurram virk
Does the same error is displaying when such form added outside of the request a quote popup ? Such error is not related to our theme but rather to the code of plugin that displays this form and validates it.
