When I installed the updates on my site, my contact us page lost its form. I am uncertain how to include the form again can you point me in the right direction, please.

Also please let me know where to remove the – WordPress DEMO Store. All Rights Reserved. at the bottom of the site, I have checked the footer static block as well as the customization pages but cannot find where to remove it. Thank you

Hello,

1. Select Contact page template in Page Attributes http://prntscr.com/gby9vj
2. Go to Appearance > Widgets > Footer Links area and add your Text widget there.
