  Hello,

  Look, you can use shortcodes of the fields that you used in your form http://prntscr.com/vk054l and place them to areas you want to show it http://prntscr.com/vk0401 You don’t have any element in your form with the shortcode [your-name] or [your-subject] so you’ll get just shortcodes.
