To create such a form, you will need to use a contact form plugin, such as Contact Form 7. If you prefer to use another plugin, for example, Cool FormKit Lite – Advanced Form Builder for Elementor, which appears to be installed on your website, please refer to that plugin’s documentation for detailed instructions.

To set up the form, you can either create a custom page and build your form there or insert the shortcode of the form you have created.
