Assistance Needed – Ads Not Showing in Header, Homepage & Post Header

I am facing an issue displaying Google AdSense ads on my website. I have implemented the following ad code, but the ads are not appearing in the header, homepage, or post header sections: please help.

The ads are sometimes showing and sometimes not. can you tell me what may be the the reason.
