  Hi, I’d like to add a contact form and wondered if you have one available in the theme, or if I’ll need to install a plugin (or add custom html, etc). Please let me know what you recommend.

  I’m just looking for a simple form, similar to the “Chat Console”, but one that I can place on a single page.

  Try to use our theme form. If you don’t like it, try the Contact Form 7 plugin.
