You can create a form using Mailchimp for WordPress plugin and add a shortcode of the form on the cart page http://prntscr.com/pi9nft > http://prntscr.com/pi9nbq = http://prntscr.com/pi9nks If this is not what you are looking for, you can contact WooCommerce support, because the Cart page is a default page of WC.
